CE FORM
Your CE form will be included in the Program Book that will be in your conference bag you received on site. The CE form is page 19 & 20 in the program book.
You simply tear out the 2 sided form, fill in the sessions you attended on the first side, then on the other side you fill in your total hours and contact information, then sign the form. Please retain the CE form for your future reference when you need to apply for re-licensure per your state. The form has already been signed by ACVC.
If you did not retain the program book and need another CE form you will need to send an email request to the office, attendee@acvc.org. Please allow up to 2 weeks for a response as we need to verify your attendance before we can send you the CE form.

The American Association of Veterinary State Boards RACE committee has reviewed and approved the program referenced above as meeting the Standards adopted by the AAVSB. This program will be included under your approved provider listing on AAVSB’s Website (www.aavsb.org).

Participants should be aware that some boards have limitations on the number of hours accepted in certain categories and/or restrictions on certain methods of delivery of continuing education.
Please note that 5.00 hours were NOT approved by RACE. Those hours specifically are for:

- Holistic and Homeopathic Treatment of Skin Problems (1 hour)
- Holistic and Homeopathic Treatment of Digestive Tract Problems and Obesity (1 hour)

The Certified Veterinary Practice Manager (CVPM) Certification
The CVPM Board does accept CE hours from the ACVC management track toward the application and renewal CE requirements. Total CE hours available will be 20 hours, 7 hours for Tuesday, 7 hours for Wednesday and 6 hours for Thursday.
To maintain certification, the Certified Veterinary Practice Manager must accumulate forty-eight (48) continuing education hours and submit documentation. Professional credits for recertification may be accumulated in a wide variety of ways. Certified Veterinary Practice Managers involved in the profession have no difficulty maintaining certification.
